Set Up Automatic Payment of Monthly Class Fees

Each student will need to have a seperate automatic billing set up.

Number of Classes is per student per week. Fees will be charged each month for 5 months,
January - May. You will need to pay the montly class fees for June at the office.

If you did not pay the monthly class fees for September when you registered you must do so at the office.

	2 CLASSES PER WEEK Student Name(required)
	3 CLASSES PER WEEK Student Name(required)
	4 CLASSES PER WEEK Student Name(required)
	5 CLASSES PER WEEK Student Name(required)
	6 CLASSES PER WEEK Student Name(required)
	7 CLASSES PER WEEK Student Name(required)
	Unlimited, includes Competition Classes Student Name(required)
